Sole Proprietorship: Enter the name (First and Last) of the primary owner.

Partnerships: Enter the name used to register with the Nevada Secretary of State

Corporations, LLCs, and Trusts: Enter the name used to register with the Nevada Secretary of State

The "Business Structure" determines how a business files income tax with the IRS. Examples: Sole Proprietorship, Partnership, LLC, Corporation, and Trust.
